Home & Community Care for Young People

The Home and Community Care Program for Younger People (HACCPYP) provides basic support services to help keep people under the age of 65 to remain living independently at home. Services are targeted to younger people who need assistance with daily living tasks due to severe health issues, mental or chronic illness and their carers. These services aim to help with daily activities. HACCPYP may be available if you are not eligible for the National Disability Insurance Scheme (NDIS). 

Eligibility 

To be eligible for this program, you must be aged under 65 years old (under 50 years for Aboriginal and Torres Strait Islanders).

To access these services, you need to be assessed and approved as eligible by an Assessment Officer. The Assessment Officer will visit you to talk about your needs and establish the level of support you need. They will develop and agree an individual service plan with you.

Referrals can be made to other health providers for other services such as food services, allied health, physio, podiatry, OT and nursing.

 

Fees

This program is subsidised by the Victorian Department of Families, Fairness and Housing and delivered by Rural City of Wangaratta. However you will be required to contribute towards the cost. The amount you need to contribute depends on your income.

These fees are updated annually.
